If you're a contractor working on a California prevailing wage project using an apprenticeable craft, the DAS 140 form is non-negotiable. It serves as official notice to Apprentice Halls that you're working on a Prevailing Wage Project that has apprentice requirements — and getting it wrong can be costly. The penalty for not submitting can be up to $300 per day per worker, more on that at the end.
Steps to File the DAS 140

1. Complete the top section with your company name, address, project address, awarding agency's address, state license number, phone number, contract execution date, and your project start date.
2. Fill in the project details: estimated journeyman hours, occupation/craft used, estimated apprentice hours, dates of utilization, and the corresponding Apprentice Hall.
3. Identify the right Halls by visiting the DIR website link on the form. Search by county and occupation— your form must go to every Hall listed, even those that don't cover your exact scope.

4. Select ONE box at the bottom:
Box 1: Signatory to a union and approved trainer — send to one Hall only.
Box 2: Paying training fees to a Hall but not approved to train — send to all Halls.
Box 3: Paying the California Apprenticeship Council — send to all Halls.
5. Submit before work begins. Never leave fields blank or write "TBD" — use estimates if needed.
Financial Penalties for Non-Compliance
Failure to submit a DAS 140 can trigger penalties of up to $300 per day, per worker classification, under California Labor Code §1777.7. Additional risks include debarment from public works for up to three years, withheld contract payments, and disqualification from future bids — a steep price for a missed form.
